Three individuals were arrested in connection with an insurance fraud scheme in Hialeah where they allegedly staged car accidents to fraudulently claim insurance payouts. The Miami-Dade County Sheriff’s Office (MDSO) conducted a raid at MO Medical Center, a clinic on W 49th Street, arresting Yisell Rojas, 39; Ofir Macias, 55; and Ivian Nieto Lazo, 37.
The scheme came to light when a driver involved in a November 2024 accident tried to manipulate the crash report by adding an additional person. After reviewing the evidence, including bodycam footage, investigators realized the damage to the vehicle didn’t match the accident’s report. The two men involved later admitted to being coached by Nieto Lazo and Rojas to falsely report the accident at MO Medical Center, where they signed therapy forms, and were told how to answer questions about their fake injuries.
During the raid, Macias, the clinic’s therapist, confessed to signing medical therapy forms for patients he never treated. Rojas was also charged with possession of a controlled substance, including cocaine.
Authorities revealed that this operation is part of a larger investigation into a network of criminals staging car accidents, recruiting fake patients, and submitting fraudulent insurance claims. Miami-Dade has implemented a pilot program to address these types of crimes, which cost the U.S. insurance industry more than $2 billion annually.
