MCMINNVILLE, Ore. — Jeanette Lynn Collins, a Certified Nursing Assistant (CNA) at Life Care Center, was arrested on July 15 after admitting to stealing and using debit cards belonging to elderly residents under her care, according to McMinnville Police.

The investigation began in August 2024 when Scappoose Police reported a resident’s credit card was stolen during a hospital stay at OHSU Hillsboro Medical Center. The stolen card was used extensively across Washington and Columbia Counties.

Following months of investigation, a Washington County grand jury indicted Collins, 58, of Lafayette, on June 13, 2025. She faces multiple felony charges, including criminal mistreatment in the first degree, identity theft, theft in the second degree, and fraudulent use of a credit card.

Authorities issued a warrant for Collins’ arrest, and she was taken into custody by McMinnville Police on July 15. In addition to the warrant charges, she faces local charges related to the thefts.

Collins is accused of unlawfully using the debit cards of two elderly residents for personal gain. The indictment includes two counts each of criminal mistreatment, burglary in the first degree, and identity theft.

Investigators uncovered additional unauthorized purchases made in Molalla, Oregon, starting July 1, 2025. While Collins admitted to taking one resident’s card, she could not recall the purchases in Molalla. Victims had not authorized any use or removal of their cards.

One victim, immobile and residing at Life Care Center from June 2 to June 19, 2025, reported her debit card stolen from her room on June 28. Unauthorized transactions totaling $382.66 were made, including purchases at a local Walmart on June 20 and 23. Walmart’s asset protection team provided video evidence identifying Collins.

When contacted at her Lafayette home, Collins was positively identified from the video and admitted to using the card “a couple of times” because she was “broke,” court records state.

Police warn there may be additional victims and have released Collins’ booking photo as part of the ongoing investigation.
